Marketing for physical therapy and healthcare clinics in Baltimore
Maryland lets patients see a physical therapist without a referral, yet most people still believe they need one. Baltimore clinics that explain direct access clearly, bring back past patients and appear in Google and AI search for specific conditions can grow without depending on physician referrals. Trident GTM builds that, starting with your existing patient list.
Direct access in Maryland, and why patients don’t know it
Maryland gives patients unrestricted direct access to physical therapy, but most patients don’t realise it. Maryland was the first state to pass direct access legislation, in 1979, and the Federation of State Boards of Physical Therapy lists its access as unrestricted. The state board tells patients plainly: “No referral needed to start physical therapy”, though some insurance plans require one for coverage.
Awareness lags well behind the law. In an APTA consumer study, 62% of past physical therapy patients and 67% of people who had never seen a PT said they would not go without a referral. A clinic that answers “do I need a referral for physical therapy in Maryland?” clearly, on its own site, reaches patients before they ever visit a physician.
The Baltimore physical therapy market
Demand is growing and the market is fragmented, which favors well-run independent clinics. The Bureau of Labor Statistics projects employment of physical therapists will grow 12% from 2025 to 2035, driven largely by an older population.
Locally, BLS counts 73 therapy offices with employees in Baltimore city, covering physical, occupational and speech therapy and audiology, and nearly 500 across the metro. Nationally the outpatient market is split across more than 38,000 clinics, with over half of outpatient PT companies earning under $1 million.
How patients choose a clinic
Patients choose clinics the way they choose any local service: they search, compare reviews and book with the one that seems most credible and easiest to reach. 97% of consumers read reviews for local businesses and 74% look for reviews from the last three months, and a growing share ask AI assistants for recommendations.
Healthcare reviews need care. Replying in public can never confirm that someone is a patient or discuss their treatment, so clinics need a reply approach that stays within privacy rules. Our guide to Google reviews covers what to say and what to avoid.
How the Local Leader Program works for clinics
For physical therapy and healthcare clinics, the program focuses on returning patients, direct-access patients and the conditions you treat best.
| Program part | What it looks like for you |
|---|---|
| Grow revenue from your existing pipeline | Campaigns for past patients due a re-evaluation or maintenance plan, follow-up for patients who stopped before finishing care, and missed-call text-back |
| Win new, high-value clients | Condition and service pages written for direct-access patients, a managed Google Business Profile, and AI search visibility for questions such as whether a referral is needed |
| Build authority in your industry | Features on health and local news sites, therapist explainer videos, and a reviews engine that respects patient privacy |
We take one clinic per service focus in each area, so we never help two competing clinics chase the same patients. See plans and guarantees.
Results from healthcare clients
Kam, a physiotherapist, reached the map results for many of the services he offers, roughly doubled his annual revenue, and hired two new therapists to keep up with demand, growing from one bed at half capacity to three beds at near full capacity. His full results are on our results page.
Questions clinic owners ask
How do I get more patients without relying on physician referrals?
Explain direct access on your website, build pages for the conditions you treat, and appear in the map results and AI answers for those conditions. Patients who learn they can book directly are the ones who arrive without a referral.
How can I reduce cancellations and patients who stop coming?
Reminders, easy rescheduling by text and a personal follow-up after a missed visit help. Past patients who stopped early are also the first group in a win-back campaign, covered in how to win back past customers.
How do I show up when people ask ChatGPT for a physical therapist?
Keep your listings consistent, earn steady reviews and publish clear condition pages that AI tools can read. The steps are in how to get recommended by ChatGPT and Google’s AI.
